Care Coordinator Job Description

Care coordinators are professionals who work with patients, caregivers, and family members.

These professionals may be involved in long-term illnesses, chronic conditions, or disabilities.

They may help patients and their families understand diagnoses, treatment options, and disease progression.

Care coordinators also may help coordinate home care services, coordinate transportation and in-home services, and provide emotional support.

Most care coordinators have at least a bachelor’s degree in social work, psychology, gerontology, or health care administration.

Care coordinators work in a number of different settings including hospitals, outpatient clinics, long-term care facilities, and home care agencies.

Job Brief:

Care Coordinator Duties:

  • Screen, assess, and refer clients to appropriate care programs
  • Assist patients in developing individual care plans
  • Investigate potential care needs and advocate for required services
  • Provide care for patients as needed
  • Assist with patient transitions between care programs
  • Track patients' health status and progress, monitoring care plans
  • Advise and educate patients on treatment choices
  • Develop and implement new care programs
  • Track, report and analyze data

Care Coordinator Responsibilities:

  • Provides a wide range of case-management services to older adults, including coordination of services, providing access to community resources, and problem-solving
  • Assist individuals and their families in dealing with barriers to care
  • Establishes and maintains communication with clients, family members, staff, and outside agencies
  • Arranges for needs assessments and care plans
  • Provides oral and written reports to management and clients
  • Manages case load and collaborates with other team members as needed
  • Participates in quality assurance and performance improvement activities
  • Perform other reasonable duties as assigned

Requirements And Skills:
