  • Assist individuals and their families in dealing with barriers to care
  • Establishes and maintains communication with clients, family members, staff, and outside agencies

  • Provides oral and written reports to management and clients
  • Manages case load and collaborates with other team members as needed
  • Participates in quality assurance and performance improvement activities
  • Perform other reasonable duties as assigned

Requirements And Skills:

  • 2+ years’ proven experience in a case management or case management-related role
