Care Provider Duties:

  • Provide direct care to clients in their homes, assisting clients with day-to-day tasks
  • Maintain accurate documentation on service visits, client progress, and outstanding issues
  • Continue education on best practices, personal skills, and program techniques
  • Ensure client safety and satisfaction
  • Do all tasks in a safe manner and follow all policies, procedures, and standards

Care Provider Responsibilities:

  • Provide counseling, mediation, and crisis management as needed
  • Perform mental and behavioral health assessments and diagnostic testing, as well as administer psychotropic medication
  • Form long-term therapeutic relationships with patients, sharing life experiences, discussing goals, and supporting patients in achieving them
  • Develop rapport with patients, attending to comfort needs, making referrals to resources, and providing referrals for specialty care
  • Provide structure and intervention to patients in crisis, and refer patients to community resources
  • Provide counseling services to patients referred from community programs
  • Provide 24-hour on-call coverage, including weekends and holidays

Requirements And Skills:

  • High School diploma or equivalent and 1 year of related experience and/or training; or equivalent combination of education and experience
  • Ability to perform a wide variety of personal care services, including bathing, toileting, meal preparation, light housekeeping, and assistance with ADLs
