Job Brief:

We’re looking for a Case Manager who can help us with our current caseload.

Case Manager Duties:

  • Communicate with patients and their family members to discuss patient needs, goals, and medical history
  • Advise patients on available resources, treatment plans, recuperation methods, and techniques
  • Schedule follow-up appointments, evaluate the effectiveness of treatment plans, and adjust treatment plans as needed
  • Acquire and record medical histories from patients, their physicians, and other health care providers
  • Maintain and operate administrative and computer programs
