Hospice Nurse Job Description

A hospice nurse is a care provider for individuals with life-limiting illnesses.

Hospice nurses provide palliative care for individuals with a life-limiting condition.

Hospice provides care that is focused on comfort rather than cure.

Hospice nurses care for patients with terminal illnesses, such as cancer or Alzheimer’s.

Hospice nurses provide care for individuals who choose to die at home.

These nurses also provide care to individuals in hospitals or other care facilities.

Hospice nurses provide care for patients at all stages of illness, including those who are facing the last days of their lives.

Job Brief:

We’re looking for a Hospice Nurse to provide compassionate care to patients and their families who are facing a terminal illness. The Hospice Nurse will work with the patient’s physician to develop a plan of care that meets the patient’s individual needs and preferences. The Hospice Nurse will also provide support to the patient’s family and caregiver

Hospice Nurse Duties:

• Provide support and care to patients at the end of life
• Perform basic nursing duties such as changing linens, monitoring vital signs, and recording information
• Deliver education to patients and family members about symptom management, pain management, comfort measures, and end-of-life care
• Demonstrate compassion and understanding when interacting with patients, family members, and staff members
• Arrange for hospice care patients to be cared for outside their homes
• Perform other duties as assigned

Hospice Nurse Responsibilities:

• Nursing degree required
• Current State Registered Nurse license
• Experience working with hospice patients
• Experience with computers

Requirements And Skills:

• Assist terminally ill patients and their families with emotional and physical support, and provide information, advice, and guidance to families, patients, family caregivers, and hospice professionals
• Manage the patient’s hospice care plan, including revising the plan with patient and family input
• Assist with activities of daily living
• Assist with terminal care and comfort measures, including educating patients and their families
• Perform assessments and evaluations
• Provide care, such as pain management, IV therapy, and hospice care
• Monitor vital signs and report changes in health status
• Providing care, comfort, and support to patients and families at the end of life
