Job Brief:

We’re looking for a Nurse Navigator to help us improve patient care and outcomes. The ideal candidate will have experience working in a hospital or clinical setting and be able to effectively communicate with patients and families.

Nurse Navigator Duties:

  • Tracking patient’s health progress
  • Helping patients through different phases of recovery
  • Ensuring that patients are compliant with medications
  • Meeting with patients and monitoring their health
  • Collaborating with physicians and staff while monitoring patient’s progress
  • Assisting patients when readmission is needed

Nurse Navigator Responsibilities:

  • Assisting patients with insurance, including the application for government and private insurance benefits, including elements of precertification and preauthorization
  • Assisting patients in understanding and managing their health care benefits and insurance coverage
  • Assisting patients with navigation of the healthcare system
  • Communicating with patients and their providers to ensure care is planned and delivered
  • Facilitating access to care, including transportation, scheduling, and referrals

Requirements And Skills:

  • Bachelor’s degree in nursing
  • Registered nurse license in good standing
  • Certification as a nurse navigator
  • Bachelor’s degree in healthcare administration
  • 4+ years’ of nursing experience
  • Clinical nursing experience in emergency department, intensive care, or critical care units is preferred
