Job Brief:

We're looking for a Patient Advocate to help us ensure that our patients receive the best possible care. The Patient Advocate will be responsible for communicating with patients and their families, as well as with our medical staff. The Advocate will also be responsible for researching and resolving any issues that our patients may have.

Patient Advocate Duties:

  • Work with patients and family members to understand their needs and develop an appropriate plan of care
  • Research up-to-date technologies and medical resources that can help patients
  • Track patients’ medical status and follow-up with physicians
  • Create and improve the company’s database
  • Assist in planning and implementing patient recovery programs
  • Manage patient’s experience and provide proactive care

Patient Advocate Responsibilities:

  • Ensure patients receive appropriate care and services, while following all policies and procedures
  • Provide comprehensive, accurate, and timely assessment of patient needs, including referrals and follow-up, and document all details in designated system
  • Maintain patient records by updating patient information and following all required standards
  • Perform education, counseling, and motivational sessions with patients, families, and caregivers to promote positive behavior changes
  • Communicate regularly with office staff, maintaining positive working relationships
  • Assist with patient billing, collections, and insurance follow up

Requirements And Skills:
