Job Brief:

We're looking for Patient Care Coordinator to help us ensure that our patients receive the best possible care. The Patient Care Coordinator will be responsible for coordinating patient care between different departments, as well as communicating with patients and their families.

Patient Care Coordinator Duties:

  • Provide general administrative and clerical support for designated department or unit
  • Complete other jobs of a similar nature
  • Hire, train, supervise, and evaluate support staff
  • Provide new staff members with job-specific orientation
  • Review and finalize procedures, policies, forms, and handbooks
  • Coordinate and maintain facility orientation
  • Supervise clerical staff
  • Ensure personnel are properly classified, trained, paid, and motivated
  • Review, approve, and implement department policies
  • Provide administrative support to the unit or department
  • Perform such other duties as may be assigned by management

Patient Care Coordinator Responsibilities:

  • Assist in scheduling patients for appointments, including verifying insurance eligibility, selecting visits, scheduling follow-up and follow-up visits, and resolving patient issues
  • Make outbound calls to new and existing patients to schedule appointments
  • Perform administrative support tasks, such as processing payments, filing, data entry, and answering phones
  • Assist with patient scheduling, including scheduling phone calls and appointments
  • Establish and maintain a rapport with patients to ensure continuity of care
  • Work with patients to confirm appointments, show up for appointments, and resolve patient issues
  • Assist providers with clinical aspects of care
  • Attend in-service training to stay current on regulatory and compliance changes
  • Establish and maintain positive relationships with patients, providers, and staff members

Requirements And Skills:

  • Bachelor’s degree or trade school certification and one year related experience
  • 2+ years’ home care experience preferred
  • Knowledge of home health care regulations, Medicare/Medicaid/insurance requirements, and medical treatments
