A Patient Navigator (PN) provides assistance to patients in the form of patient education and navigation of healthcare resources.

Patient Navigators also provide support to patients that face challenges navigating the healthcare system, such as insurance, transportation, and language barriers.

Patient navigators work in a variety of settings, such as hospitals or clinics, community health organizations, transitional care programs, and home health agencies.

Patient navigators work to improve access to care, lower healthcare costs, facilitate social integration of patients, improve coordination of care, and reduce health disparities.

Patient navigators work in roles ranging from entry level to advanced.

Job Brief:

We’re looking for a Patient Navigator who can help us guide our patients through their treatments and help them understand their options.

Patient Navigator Duties:

  • Advise patients about all aspects of their care (medical, surgical, and procedural)
  • Assess patient’s readiness to change behaviors or treatment plans
  • Assist patients in finding and scheduling appointments
  • Provide patients with information about services, referrals, or alternative services
  • Provide patients with information about local community resources
  • Provide patients with information about community support groups
  • Assist patients in developing skills to cope with their health issues
  • Increase access to care by educating patients about available services
  • Provide patients with information about financial options
  • Provide patients with information regarding insurance, benefits, or financial assistance
  • Maintain and document correspondence for patient records

Patient Navigator Responsibilities:

  • Work closely with clinicians and patients for their assistance with accessing healthcare services
  • Screen incoming patients to identify appropriate admission and discharge criteria and monitor hospital discharge processes
  • Work with patients to determine their healthcare needs
  • Provide patient education and support, including coordinating pre- and post-discharge services
  • Utilize appropriate discharge planning practices and procedures, and hospital discharge processes
  • Provide administrative and clinical support
  • Administer medical tests, medications, and treatments

  • Proven experience in a healthcare setting
  • Strong decision-making capabilities
  • Above-average communication, collaboration, and delegation skills
  • Proven ability to develop and maintain financial plans
  • Ability to motivate and lead people, and hold employees accountable
