We’re looking for Registered Nurse (RN) Case Manager to join our team and provide high-quality patient care. As an RN Case Manager, you will be responsible for coordinating and managing patient care, as well as providing support and guidance to patients and their families.

Registered Nurse (RN) Case Manager Duties:

  • Provide and assess care to the patients
  • Ensure patient medical records are up-to-date and accurate
  • Research and monitor patient medical histories and medication lists
  • Conduct assessments to determine patients’ medical needs and prognoses
  • Provide education and support regarding patient diagnoses and treatment plans, as well as personal and family concerns
  • Participate in care coordination by providing referrals and assisting with coordinating care
  • Monitor and report patient’s progress
  • Maintain strict confidentiality
  • Ensure medication is administered by qualified staff
  • Maintain accurate documentation of all patient records
