We’re looking for Patient Care Coordinator to help us ensure that our patients receive the best possible care. The Patient Care Coordinator will be responsible for coordinating patient care between different departments, as well as communicating with patients and their families.

Patient Care Coordinator Duties:

Requirements And Skills:

  • Bachelor’s degree or trade school certification and one year related experience
  • 2+ years’ home care experience preferred
  • Knowledge of home health care regulations, Medicare/Medicaid/insurance requirements, and medical treatments
