Social Worker Duties:

  • Help individuals, families, and communities deal with the effects of mental illness, substance abuse, and other behavioral problems
  • Provide ongoing treatment, education, and support
  • Inform individuals, their families, and caregivers about diagnosis, treatment, and services available
  • Educate individuals, their families, and caregivers about treatment, medications, and other treatments
  • Attend appointments with individuals, their families, and caregivers
  • Provide emotional support and counseling to individuals and groups
  • Advise individuals, their families, and caregivers about community resources
  • Consult with individuals, their families, and caregivers to assess their needs and plan care
  • Prepare and maintain reports and records

Social Worker Responsibilities:

  • Perform day to day clinical social work duties in outpatient mental health setting
  • Maintain accurate and timely documentation in accordance with all regulatory standards and requirements
  • Assist in obtaining and monitoring service plans for clients, to include housing, employment, and treatment/recovery services
  • Assist with discharge planning
  • Perform intakes and assessments, and determine need for treatment
  • Develop and implement treatment plans, and document progress notes
  • Perform and document home visits
  • Assist with crisis management
  • Support community referrals, including crisis intervention, grief/loss, and substance abuse

Requirements And Skills:

  • Bachelor’s degree in social work, psychology, or a related field
  • 2+ years’ relevant experience
  • Highly organized, multi-tasking ability and a commitment to confidentiality
  • Skills in conducting psychosocial assessments
  • Strong management skills, including the ability to delegate
  • Ability to communicate effectively
