Job Brief:

We’re looking for a Home Health Nurse to work with us in providing care to patients in their homes. The Home Health Nurse will be responsible for assessing the patient’s needs, developing a care plan, and providing nursing care to the patient.

Home Health Nurse Duties:

  • Actively participate in assessing and evaluating patient care needs, developing and administering nursing plans of care, and executing treatment plans
  • Develop, implement, and evaluate nursing plans of care
  • Perform nursing assessments, including health history, physical assessment, and mental status
  • Implements nursing interventions and collaboration with physicians
  • Monitors patient’s response to treatment, and adjusts care as needed
  • Assist with transporting patients’ to and from appointments
  • Educate patients and their families in healthcare-related information
  • Support patients’ physical and emotional needs
  • Communicate with other members of the health care team, such as physicians, pharmacists, social workers, and technicians, regarding patients’ progress
  • Assist with developing and maintaining detailed patient records, including charting, correspondence, and reports
